By signing below, I certify and agree that: (1) I am the named cardholder or have lawful authority to use the card and bind the customer/company identified above; (2) I requested, approved, received, and accept the services, labor, diagnostics, parts, towing/recovery, mileage, call-out, wait time, storage, taxes, and other itemized charges shown on the related work order or invoice; (3) I have inspected the work or had a reasonable opportunity to do so, the work and service are satisfactory and complete for the authorized scope, and the vehicle/equipment has been returned, released, delivered, or handled as directed by me, except only for written exceptions stated below; (4) I authorize Best Rate Fleet Services to charge the amount shown above and understand the statement descriptor may identify Best Rate Fleet Services or its payment processor; (5) I understand that diagnostic conclusions, temporary repairs, customer-declined recommendations, pre-existing conditions, and work outside the authorized scope are not promises that the vehicle/equipment is free of all defects; (6) I will not falsely claim that this transaction was unauthorized, fraudulent, duplicated, that services were not received, or that the work was unsatisfactory when those claims are contradicted by this signed acknowledgment, the invoice, photographs, communications, location records, processor data, or release records; and (7) I will promptly contact Best Rate Fleet Services to allow a good-faith investigation of any claimed billing or service error. Nothing in this form waives any right that cannot lawfully be waived.
